For patients, however, regulation and clinical evidence remain separate questions: a licensed facility does not automatically validate every benefit claimed for a particular treatment.

🔬 What Benefits of NAD+ Are Being Studied?
Research interest spans energy metabolism, physical function, metabolic health and brain health.
⚡ Energy, Fatigue and Physical Function
Because NAD+ is central to cellular energy metabolism, researchers have investigated whether increasing NAD availability can affect fatigue and physical performance.
Some studies of NAD precursors have produced encouraging signals.
A randomized study of 108 older adults found that 12 weeks of oral NMN was associated with improvements in selected lower-limb function and drowsiness measures, particularly in the afternoon-treatment group.⁴
Another randomized trial found that 12 weeks of NMN increased blood NAD-related metabolites and improved a secondary four-meter walking-time measure, although the study's primary stepping-test endpoint was not significantly different from placebo.⁵
These results support continued research. But both involved oral NMN, not IV NAD+.
🧠 Cognitive Health and Mental Clarity
NAD metabolism is relevant to brain energy metabolism, which helps explain the interest in NAD+ for focus, cognition and “brain fog.”
Human results are not yet consistent.
In a placebo-controlled study of older adults with mild cognitive impairment, NR increased blood NAD+ substantially, but measured cognition did not improve during the trial.⁷
That does not rule out future applications. It demonstrates why increasing a biomarker and improving how someone thinks, feels or functions should be treated as different outcomes.
🩺 Metabolic Health
Metabolic research has also produced promising findings.
In a randomized trial of overweight or obese postmenopausal women with prediabetes, 10 weeks of NMN improved muscle insulin sensitivity and insulin signaling.⁶
The study is clinically interesting, but it involved a defined population receiving NMN.
It should not automatically be interpreted as evidence that a healthy person receiving IV NAD+ will experience the same effect.
📊 What Does the Human Evidence Say About IV NAD+?
The strongest human evidence currently comes from the broader field of NAD augmentation rather than direct IV NAD+.
A 2026 systematic review identified 113 eligible studies, including 33 human intervention studies and 28 randomized human trials.
Oral NR and NMN consistently changed NAD-related biomarkers, but effects on physical, metabolic, vascular and other healthspan-related outcomes were heterogeneous and often endpoint-specific.³
Most importantly, the review found no eligible clinical-outcome trials of IV or intramuscular NAD+ itself for anti-aging or wellness indications.³
📋 NAD+ Evidence at a Glance
Direct IV research is beginning to fill this gap.
A 2019 pilot administered NAD+ over six hours and examined changes in NAD and its metabolites in blood and urine.
It helped clarify what happens after infusion, but it was not designed to test improvements in energy, cognition or longevity.⁸
A 2026 retrospective pilot compared four consecutive days of 500 mg IV NAD+ with 500 mg IV NR.
It generated useful preliminary information on infusion tolerability and short-term biomarkers, but the authors concluded that further research is needed to establish dosing and effectiveness beyond 30 days.⁹
🕰️ Can NAD+ IV Therapy Slow or Reverse Aging?
Direct IV NAD+ has not yet been shown in robust human trials to reverse aging or extend lifespan.³
That does not make the field unimportant.
